Will I be able to achieve a full recovery?

Yes, Most patients are able to achieve full recovery and function after an ACL reconstruction Surgery.  The only difference is their recovery timeframe:

  • Immediately after the operation, the patient is allowed to walk with crutches. 
  • By the 1st to 2nd week after surgery, the patient usually can go without crutches.
  • Most patients can start light running at 4 to 6 months after the ACL Tear surgery. (In terms of resuming intense sports, it is recommended to wait till 9 months to one year after surgery.)
  • Regardless, all patients after an ACL reconstruction Surgery will feel that their knees are stronger and more stable compared to before surgery.

Pre-op physiotherapy is recommended to achieve some muscle memory in the relevant exercises and maintain knee mobility as these will facilitate post-op rehabilitation. You will be required to fast for six hours prior to the timing of ACL tear surgery, and avoid applying plasters, topical medications, kinesiotape or knee guards over the knee in case you develop skin allergies to these materials.

  • Expect to undergo a thorough evaluation before the surgery, including physical examination and imaging tests such as MRI.
  • At  ACL Reconstruction Singapore, you will be given general anaesthesia, during the surgery to ensure you are comfortable and pain-free.
  • Once this has taken effect, The procedure will involve making  2 small incisions over the front of the knee to do an arthroscopic examination of the knee and to remove the damaged ligament. A tendon, known as a graft, is either harvested from another region of the same knee, or is taken from a donor (also known as an allograft), and this will be used to replace the torn ACL ligament. The graft is then secured to the femur bone and tibia bone with titanium buttons and bioabsorbable screws. These implants are MRI compatible and you will still be able to take MRIs after the surgery.
    • At the end of the surgery, local anaesthesia will be infiltrated around the wounds or a nerve block might be given near the knee to optimise postop pain control. Waterproof dressings will be applied over the wounds.  A shower cover would be also provided so that you can shower with no fear of getting the dressings wet or causing them to fall off. A rigid knee brace and crutches will be provided before discharge.

It is important to follow your surgeon's instructions regarding weight-bearing restrictions and your rehabilitation period lifestyle. Otherwise, the following complications can arise:

  • Activity limitations
  • Bleeding 
  • Mild knee pain.
  • infection.
  • Knee stiffness or 
  • Weakness.
  • loss of range of motion.
  • Occasional soreness in the knee

It typically takes almost six months to fully recover after ACL reconstruction surgery, and get back to your pre-injury condition with complete range of motion and stability in the knee joint.

  • Recovery time can vary from patient to patient. After the surgery, you will be asked to come back to the clinic to have your dressings changed every few weeks or a week, till dressings are allowed to come off at two weeks post-surgery. Twice weekly physiotherapy is recommended for a start and this will help reactivate the muscles and reduce the swelling.
  • With physiotherapy and rehabilitation, most of the patients can return to competitive sports in 1 years on average after an ACL surgery.
  • At 4-6 months postop, the sense of balance and control of the knee should be sufficient for the patient to start some light jogging. With a tight postop physiotherapy regime, competitive play might be possible at 9 months postop.
  • Office-based work can be resumed once the dressings come off at 2 weeks.

The first 2 weeks are critical in your healing process after ACL tear surgery and you should start physiotherapy as soon as possible. It is imperative to obtain a full extension as an initial priority.  After the second or third week, you should be able to move without the use of crutches.

If your ACL is partially torn, a widely recommended approach for initial management involves the utilization of the RICE method. RICE stands for:

  • Rest: Giving the injured knee the necessary time to heal by minimizing any weight-bearing or strenuous activities
  • Ice: Applying ice to the injured area to constrict blood vessels and decrease blood flow to the affected region.
  • Compression: The application of compression to provide support to the injured knee while also helping to minimize swelling.
  • Elevation: Elevating the injured knee above heart level serves to further reduce swelling and encourage fluid drainage away from the site of injury.

Keep in mind that, It is common to experience swelling, pain, and limited mobility in the initial weeks following surgery. Pain medication may be prescribed to manage discomfort. Also, you can regain rapid strength and stability in your knee with proper rehabilitation exercises.
